UnifiedCARE For organizations in duty of care

We seek to address some of the challenges in healthcare through communication technologies. We do this via our innovative collaboration platform called UnifiedCARE®. It enables efficient communication between the physicians, nurses, and any party involved in patients' care. UnifiedCARE provides both web and mobile applications with the following capabilities:

Care Transparency
ActiveMEDI is a digital solution both for businesses and individuals, or any organization in duty of care. It helps you to manage communications across the continuum of care at home, clinics and hospitals and bringing transparency towards the involved parties.

Care management
Establish the outstanding and completed tasks for all patients for example in home care visits or in hospital, and enable efficient channel to report and communicate directly with relevant parties.

Easy start

Support care providers to easily run a customer-focused care with less hassle for setting up the care planning system. UnifiedCare is an easy service activated via few clicks only within few minutes. No pre-investment or hidden cost, pay for what you use.

Care continuity
Extend the traditional care outside your care facilites. Discover the important information about patients and instantly share new information relevant to other parties in duty of care.

Remote Monitoring
Securely and reliably collaborate between wards, and away from the hospital and follow-up patient condition with direct communication with the patient and care providers. Notify/alert nurses and family members when needed.

Integration

We have already done successful integration with nation-wide PHR systems via strong authentication (personal bank authentication). ERP or API-level integration can be activated easily with full read-write capabilities after a customer request.

About us

Company details

We are a Helsinki-based Finnish company that focuses on innovative healthcare solutions, providing best-in-class online services and tools for an active life style and wellbeing. ActiveMEDI was founded by a former Nokia business director, Alireza Hasanpour.

Alireza Hasanpour
Alireza is an expert in mobile, consumer, and enterprise technologies with 22 years of experience. During his carrier he has been involved in early stage start-ups and companies and developed innovative solutions, for banking, insurance, etc. He developed the first online banking service for consumers in Oslo, Norway with best-in-class security standards.

Alireza worked in various organizations at Nokia (CIO office, Services, Markets strategy and Corporate development units) and was involved in latest Wellbeing strategy initiatives. In the summer of 2012, his father got ill and needed intensive care, not only from care professionals, but also the entire family. The biggest challenge was managing his daily care and communicating with the health professionals.

Living away from his father, and his Nokia background, encouraged him to further seek a solution for the challenges. This was the birth of ActiveMEDI, a solution to the current institutional and reactive healthcare system.

To make ActiveMEDI a reality and fill the gap in the market, Alireza needed to find the best talent and develop a good team. In January 2013, he found Jonathan Wiklund that was a perfect match for his vision.

Jonathan Wiklund
Jonathan had recently come back from an exchange year in the US, where he was struggling with back-pain due to two herniated discs. While spending time in California he received good care for his back, but as soon as he came back to Finland he realized that the communication between doctors about his care was not working as it should.

Speaking to Alireza about his vision and plans for families and the management of care, it quickly became apparent to him that this was the missing piece of the healthcare systems.

Jonathan is a young energetic software engineer that has been working since then developing the ActiveMEDI service. As a co-owner and CTO, he has a central role in the development and currently takes care of the product development.
