Spinal anaesthesia

In the 1970s PAJUNK® revolutionised spiral anaesthesia with the SPROTTE® cannula, a joint development with Prof. Sprotte. Due to the atraumatic tip geometry it was the first puncture cannula that reduced all side effects of spinal anaesthesia to a minimum, making it an alternative for general anaesthetic.

These atraumatic quality features lead to outstanding gliding properties, minimise injuries during puncture and prevent the introduction of foreign objects and tissue particles into the spinal space. The time required between puncture and injection of the anaesthetic is markedly reduced, because the geometry of the lateral eye encourages the free flow of cerebrospinal fluid, which is also immediately recognised by the user thanks to the special cannula hub.

The Lumbar puncture is an indispensable instrument for diagnostics and therapy control of inflammatory and oncological diseases of the central nervous system. The PAJUNK® atraumatic SPROTTE® cannula sets standards for the security of the invasive intervention.

Characteristics of the SPROTTE® cannula
 
  • The closed, ogive-shaped tip displaces tissue during puncture rather than cutting it, as is the case with Quincke cannula tip
  • The lateral eye has rounded edges

NRFit – New ISO standard

For more patient safety
As a leading manufacturer of regional anaesthesia products, PAJUNK® sees its responsibility to advance the ISO standard 80369-6, which is supposed to be adopted in 2016 as an international norm. This standard defines new requirements for small bore connectors in the field of regional anaesthesia to prevent the risk of misconnections of different supply lines to the patient. This measure is aimed to reduce the risk to of accidental misconnections and injections thus improving patient safety. The change will be designated by the new name: NRFit.

The NRFit standard is currently being extended in the fields of peripheral nerve blocks, wound infiltration, and lumbar puncture.

Optimised hub with NRFit connector
All products concerning procedures in the field of regional anaesthesia will be equipped with the new NRFit connectors. The procedure remains the same, but safety is improved. Improved magnifying function: enlarged magnifying glass in the hub permits a faster detection of the liquor.

Ultrasound guided regional anaesthesia

Ultrasound guided regional anaesthesia has become the dominant technique. Today even the finest anatomical structures including peripheral nerves can be identified in detail and anaesthetised selectively under viewing, using the modern high-resolution ultrasound scanners. Echogenic cannulas are more convenient, contribute to an improved quality of the ultrasound image in use, when compared with non-echogenic designs1 whereby the cannula is advanced completely under viewing, resulting in a minimization of the risk of complications. The visibility of cannulas under ultrasound monitoring has become one of the most important criteria for the selection of a product in clinical practice, particularly in steep insertion angles. 2Based on the „Cornerstone“ technology , developed together with Dr. Chris Mitchell, PAJUNK® launched the echogenic Sono cannulas a few years ago, that exactly meet these requirements.3

Wound infiltration

Wound infiltration is in fact an extremely effective method for post-operative pain treatment, which is very simple to apply. A local anaesthetic is continuously administered by way of a catheter with multiple perforations, which is positioned in the wound, thereby blocking the distal nerve ends. Aside of the purely anaesthetic properties, some local anaesthetic also has an anti-inflammatory and antibacterial effect. 

The great advantage of this economical method is, that the post-operative occurrence of side effects such as nausea and vomiting caused by opioids is reduced. This optimized form of pain therapy is an essential component of all treatment paths for fasttrack rehabilitation. 

Based on decades of experience in manufacturing catheters, PAJUNK® has developed InfiltraLong as a special wound infiltration catheter for the continuous treatment of long and deep wounds after operations. The special characteristics of the catheter permit the consistent and precise, on-target administration of local anaesthetic. 

There are two infiltration sets available:

  • with a split cannula and optional elastomer-pump
  • or with a Tuohy cannula
Freedom from pain along the complete lengthIn the development of the InfiltraLong, PAJUNK® has combined decades of experience in the manufacture of catheters with the knowledge concerning the special requirements for pain treatment of large wounds. The main issue is the precise and uniform administration of the anaesthetic. The InfiltraLong catheter is substantially different from other conventional wound infiltration catheters with regard to the following features:


The patented “Cornerstone” reflectors with their three reflective surfaces are crucial for the outstanding reflection of the ultrasound waves and associated with this the visibility of the Sono cannulas.3

InfiltraLong of PAJUNK® is the only wound infiltration catheter which has been provided with an integrated stainless steel helical coil, that will guarantee a uniform flow of the anaesthetic and protection against creasing or buckling during the placement of the catheter. Besides it is radiopaque and visible under ultrasound.
The hub of the catheter has been provided with an additional kink-proof protection. This will also warrant the operational safety of the catheter outside of the wound.

Hernioplasty

The PAJUNK® balloon systems are ideally suited to the endoscopic totally extraperitoneal hernioplasty (TEP). With this gentle method, the preperitoneal space is dilated pneumatically by inserting and inflating a balloon and the abdominal wall is reinforced with a plastic mesh. Not only the risk of infection, but also the size of the incision is significantly reduced, and at the same time the necessary recovery time after the operation is minimised. 

The balloon systems together with the trocar systems offer you a modular kit of PAJUNK® instruments with all you need for extraperitoneal surgery. 

The medical arguments speak just as much in favour of the PAJUNK® balloon system as the economic arguments.

Main advantages of the TEP method:
  • No opening of the peritoneum, hence no risk of damaging abdominal organs
  • No mesh fixing with clips necessary
  • Ideally suitable for relapse hernias

The cholangiography forceps – made of a very sturdy allmetal construction – are used to grasp the bile ducts, as well as for the placement of a catheter for intra-operative imaging of the gall-bladder or the bile ducts. The contrast medium is injected by means of a cholangiography catheter with a LuerLock- connector, which is introduced through the channel of the forceps.
